Parent Dedication Application

Parents who wish to participate in a Parent Dedication ceremony at College Park Church must complete a request form below or print it and return the completed form to Tammi Gant's mailbox.

The next dates for 2010 parent dedications are:

         We are requesting to participate in a Parent Dedication Ceremony at College Park Church. We understand we will be making a commitment before God and the church body to raise your child in the nurture and admonition of the Lord.

      1.  Were there any special or unusual circumstances regarding your child's birth or adoption? 
      2. Why do you desire to participate in the parent dedication ceremony? 
      3. Do you feel this ceremony changes your child's relationship to God in anyway? 
      4. Do you promise to bring up your child in the nurture and admonition of the Lord? 
    

      
5. Check the activities you intend to incorporate into your child's upbringing:       Regular Sunday School and Sunday church attendance (Hebrews 10:25)
      Family times of prayer and Bible reading (Joshua 1:8; 24:15; Ephesians 6:4)
      Biblical discipline (Proverbs 19:18; 22:15; 23:13-14; 29:15-17)
      The example of a godly life (Ephesians 5:1; Titus 2:7-8; I John 2:6)



      6. A baton is given to each family with a letter from a pastor.  These are to be given to your child on their 18th birthday for them to read.  Are there any particular scriptures or biblical principles you would like expressed in the letter?
